To request an appointment for physiotherapy services, you must have a user name and password and fill in the booking form.

There is also a contact form for Quirónprevención.

The categories of personal data that we process are listed below:

• Website access credentials: User-email address, password.

The data that we request from you through the aforementioned forms are of an identifying nature and are the minimum necessary to be able to provide you with the services you request from us. The obligatory data are specified on the form itself, and your refusal to provide them will mean that we will not be able to deal with your request.

• Booking of physiotherapy appointments and access credentials: In order to manage the appointment for the physiotherapy service contracted with Quirónprevención.

The legal basis that legitimises the processing of your personal data in the booking of physiotherapy appointments and access credentials is the fulfilment by Quironprevención of the physiotherapy service contract in which you are an interested party.
